In our 19 years experience in system safety and software safety we have =
witnessed a great deal in the sectors of medical devices, nuclear =
energy, trans****tation, avionics and defense.
Our venture into system safety and software safety began with the =
THERAC-25 cancer therapy machine being consultants to the manufacturer =
following the horrible accidents in Tyler, Texas. This was to be our =
first forensic analysis. Others were to follow.
We have experienced a mixture of system safety and software safety =
programs - some good, many bad, and some that started off good and =
deteriorated along the way.
We have taught system safety and software safety both domestically and =
overseas for close to 15 years.
After this long history, it is regrettable what we are witnessing here =
in the U.S. and even worse in Canada. It is nothing short of a mess. =
System safety is in serious jeopardy in many situations in both industry =
and government. For many systems, it is too late. The safety of these =
systems will reveal itself in the years to come. =20
It is abundantly clear to us though what needs to be done to remedy this =
situation. It is not one thing. It is many things. =20
